As instructed by the appropriate authority please brings proof of your residence. Photocopy of any one of the following document which carries photograph of her or her husband.
1. Ration card (of patient/husband)
2. Voter card (of patient/husband)
3. Passport copy ( of patient/husband)]
4. Driving License (of patient/husband)
5. Bank pass book
Also bring referral Slip from a Qualified Doctor (BAMS/MBBS) in which proper indication for the Scanning should clearly be mentioned.
